Rhinoplasty or nose job is a surgery performed to change the shape of your nose. 

1. In which situations should nose aesthetics be performed?

Rhinoplasty is an operation applied both visually and in terms of health. In general, aesthetic nose surgeries are performed because the shape of the nose is not liked visually. Apart from this, nasal aesthetic surgeries are also performed to correct the deformity that develops due to trauma. Rhinoplasty can be performed due to complaints such as arched nose, long nose tip, size of the nostrils or asymmetry.

2. Is there an age limit for rhinoplasty?

As it is foreseen that the musculoskeletal system will complete the adequate development in nasal aesthetic surgeries, the bone development of children is expected to be completed. In nasal aesthetics, not the age limit, the development of the bone and cartilage structure is based. However, prominent ear surgeries, which complete their development faster than the nose, can be performed even at the age of 6, considering the psychology of the child. Although there is no exact upper limit in plastic surgery, it should be considered that anesthesia and postoperative complications may increase in advanced ages. However, aesthetic surgery can be performed if there are no different conditions that prevent the patient from receiving anesthesia by looking at the general condition of the patient.

3. Is there a difference between male rhinoplasty and female rhinoplasty?

There are aesthetic differences between male and female nose. The criteria for this is usually the angle of the nose with the lip. While it is about 90 degrees for men, this angle is slightly higher for women. That is, a woman's nose should be raised compared to a man. Surgical principles applied in male or female rhinoplasty in plastic surgery are the same; however, the size and the amount of angles vary here. The tip and width of the nose is more pronounced in men. The nasal dorsal and nasal passage should be unclear and the nose-lip angle should not look feminine.

4. How should the rhinoplasty doctor chosen?

The person who will have a rhinoplasty usually makes a choice between a plastic surgery specialist and an ENT specialist. Functional surgery of the nose is mainly in the field of otolaryngology; Although nasal aesthetics seems to be entering the field of plastic surgery, it can be performed by experienced surgeons by both branches. The patient who decides to have a rhinoplasty should give an idea about what he wants to change in his nose, and the doctor should give the patient an idea about what he can change. In this regard, the physician should use the available technological 2-dimensional and 3-dimensional visual tools to show the patient how the postoperative appearance will be approximately. A plastic surgeon can also solve problems with breathing in the nose. Aesthetics of the nose are more complex than functional surgeries. Both branches can handle two processes. Here, the patient's preference is important.

5. Can concha hypertrophy and nasal deviation operations be performed simultaneously with nasal aesthetics?

Structures called concha, in other words, provide the heating and moistening of the air inhaled through the nose. The enlargement of the nasal concha, namely concha hypertrophy, can also be confused with diseases such as adenoid and septum deviation among the public.

In the nasal cavity, the nasal concha in three rows from front to back on the side walls, especially those on the lower part, may grow due to allergic reactions or secondary to septum deviation. Problems that cause nasal obstruction such as nasal concha hypertrophy and septum deviation are problems that can be corrected simultaneously with nasal aesthetics.

6. Can nose aesthetics and prominent ear surgery be done at the same time?

Rhinoplasty can be performed with prominent ear and other facial aesthetics operations at the same time, depending on the physician's preference.

7. Can aesthetic surgery be applied to noses with big dorsal hump?

  In patients with big dorsal hump, if the excess bone and cartilage forming the hump of the nose is rasped down or removed, the part that we technically call the roof is opened, the roof is broken with nose cutters, piezo electric systems or chainsaws to close the roof. Recently, noses with big hump can be corrected with atraumatic rhinoplasty techniques that completely reduce the nose height without rasping down the hump in patients with appropriate hump height.

8. What is nasal tip aesthetics (tip pasty)?

Nasal tip aesthetics or tip plasty  is a surgery in which the cartilages at the tip of the nose are shaped.  The nose and lip angle can be increased and decreased by shaping the cartilages at the tip of the nose. The tip of the nose can be thinned. While nasal tip aesthetics are performed, the nostrils can also be intervened. This process; It is a preferred method for patients who do not have a dorsal hump but have a low nose tip, have problems with their nostrils or have a very wide nasal tip.

9. Are nose reduction surgeries difficult?

Rhinoplasty surgeries are generally not very painful operations. However, one of the most common mistakes about rhinoplasty surgery is the knowledge that nasal plastic surgery is painful. Because of the bruises and swelling that occur after the surgery, people believe that nasal aesthetic surgeries are painful and troublesome operations. In the past, the nasal aesthetic tampon relationship frightened people. Problems experienced during the removal of a tampon after nasal aesthetics have left its place to nasal aesthetic surgeries without a tampon. Since the tampons used in the past were of a cotton structure, they adhered to the nose and caused bleeding. Silicone tampons used today do not cause such problems. Swelling and bruising after aesthetic nose surgery varies according to; 

1-The bone and cartilage structure of the patient

2- Age , gender

3-the surgical technique used

From plastic surgeon’s point of view, one of the most difficult operations in plastic surgery is rhinoplasty surgery. Compared to other aesthetic operations, since the nose is an organ that can be seen and evaluated by everyone, its results can be seen and evaluated by everyone.

Technically speaking, rhinoplasty is a difficult aesthetic operation. Of the person; skin, bone and cartilage structure differences can make the surgeon's job difficult. Patients with thin and delicate skin, bone and cartilage structure should be treated more carefully.

In cases where the nasal skin is very thin, all kinds of technical errors can be easily noticed from the outside. Cartilages that are not sufficiently supported can also cause asymmetries during the healing process.

Likewise, plastic surgery of patients with thick skin is difficult. Beautiful aesthetic lines are less visible from the outside in patients with thick skin. The difficulty in nasal aesthetics is due to the difference in the bone, cartilage and skin structure of the patient rather than the dorsal hump or the deviation  of the nasal bone.

10. Can septum deviation be treated by plastic surgery?

What we call septum is the part that divides the nasal cavity into two, the front part is made of cartilage and the back part is made of bone and is covered with a cover we call mucosa. Septum deviation, that is, the inclination of this cartilage or bone part in the midline of the nose to one side is one of the causes of breathing disorders. The deviation here can be front or back. If there is a deviation on either side, the passage on that side is narrow and causes difficulty in breathing.

Nasal contours and septum curvatures can be operated by a plastic surgeon simultaneously with rhinoplasty.

11. How is rhinoplasty performed, which techniques are used in surgery?

In general, the techniques used in rhinoplasty are divided into open technique or closed technique. Surgeries performed with open and closed techniques have advantages and disadvantages depending on the situation.

  • Open rhinoplasty; It is performed by opening the nostrils with a small incision from the bottom. Opening this part provides the surgeon with a wider viewing angle. In general, the procedures performed on cartilage and bone are the same as the closed technique. Open rhinoplasty technique can be preferred in patients with advanced deformity or a history of a previous aesthetic operation, which we call secondary surgery. The biggest advantage of the open rhinoplasty technique is that it provides the surgeon with a wide view and application opportunity. However, the recovery time of nasal tip edema and bruises is longer than the closed rhinoplasty technique.

  • Closed atraumatic rhinoplasty; In closed rhinoplasty, the incisions are inside the nostrils. Although the surgical angle of view is less in the operations performed with the closed technique, there is no possibility of a suture scar seen from the outside. In addition, the recovery time is relatively shorter than the open rhinoplasty method.

12. Can rhinoplasty be performed under local anesthesia?

Rhinoplasty operations can be performed under general anesthesia or technically under local anesthesia. While rhinoplasty is performed under local anesthesia, it should be considered that being awake may cause different problems. The patient's restlessness, fear, and the surgeon's uncomfortable operation reduce the quality of a fine surgery such as rhinoplasty. Performing nasal aesthetic operations under general anesthesia is more comfortable for the patient and the surgeon.

13. Is rhinoplasty possible without surgery?

Non-surgical rhinoplasty presents some problems in itself. The most commonly used methods in non-surgical rhinoplasty are as follows.

  • Filler rhinoplasty (non-surgical nose job)

It is a non-surgical rhinoplasty method that can be applied in non-obvious nasal humps. Although it seems to be an advantage for patients who do not want to undergo surgery, the effects are not permanent. In cases with a nasal dorsal hump, filling the nasal root with filler can be made to make the nose appear flat, and fillers can be applied in the irregularities on the nose tip or back. Complications related to filling materials should be shared with the patient within the risks of this technique. Although the reliability of the filling materials used today is better than before, the risks and surgical complications that may be encountered are not such that they can be underestimated.

  • Nasal tip botox

Botox can be applied intramuscularly to the tip of the nose, which is the junction area with the lip. As the muscle relaxes with the effect of botox, a relative rise is provided at the tip of the nose. As with nasal botox, the effect is short-term, but the results in terms of applicability and patient satisfaction for each patient are at the discretion of the person.

  • Nose lift using threads (non-surgical nose job)

It involves inserting a piece of polydioxanone (PDO) threads through the tip of the nose to lift the tip of the nose and straighten and refine the bridge. My personal opinion is doubtful that it provides long-term permanence as in other applications.

14. Can only nasal tip lift aesthetics be performed?

Not only can the nose tip be lifted, but also operations such as reducing the wide nose tip can be performed. In such procedures where the nasal dorsum is not intervened, the height of the nasal tip, the angle of the lip and the shape of the nasal tip are corrected.

15. What is nasal dorsal hump removal?

It is a method used to reduce the height of the dorm of the nose, usually in noses with hump. With the rasp used in nose aesthetic surgeries, the back height of the nose is brought to the desired level. In cases where the nasal hump is too high, the excess part can be removed by rasping down or cutting according to the doctor's preference. However, it is a more controlled process to reduce the hump to the desired level. In cases where the nasal hump is too high, it may not be enough to rasp it. In some cases where the hump is taken up to the desired level, the nose ridge (roof) may remain open. In such cases, the side wall bones of the nose are broken inward and a roof is formed again. However, in non-high hump noses, it can be reduced to the desired level by simply rasping without this.

16.What is the most attractive nose shape?

The harmony of proportions between parts of a whole in art has an aesthetic result. This ratio is mathematically called the golden ratio. Golden ratio; It is observed in many living things of the body and nature, such as between the arm and forearm, and the adjacent finger bones. In rhinoplasty, it is tried to be adhered to this rule, since it is more welcomed in aesthetic perception to achieve rates close to these rates.

While taking care to preserve the golden ratio in rhinoplasty based on the wishes of the patients, the existing variations are also taken into account.

In order to explain this to the patient better, aesthetic surgery specialists can show the planned nose with various 2-dimensional or 3-dimensional applications. The patient and the doctor can determine the final shape of the nose to be made by exchanging ideas on these applications.

Although the wishes of the patient have priority, it is necessary to act by considering the proportions of the face. Making a very large nose too small can create problems for the skin to adapt to such an operation and may not produce satisfactory results in terms of desired results. It is important for the predicted results in the virtual environment to be in line with the postoperative results and to try to be realistic in terms of the trust relationship between the patient and the physician after the operation.

17. Can those who suffer from sinusitis have rhinoplasty surgery?

Although there is no problem in patients with sinusitis to have rhinoplasty surgery; It is more appropriate to operate patients with sinusitis complaints after eliminating the conditions that cause sinusitis and can be corrected by surgery.

18. What should the patient pay attention to before rhinoplasty?

Since very sensitive procedures are performed in rhinoplasty, it is desired to have as little bleeding as possible. The most important part of rhinoplasty is that it creates a bloodless environment. This environment is created by using special medications during aesthetic surgery. However, there are situations that the patient should pay attention to before the operation process.

  • Before the operation, you should inform your plastic surgeon and anesthesiologist all the details about the drugs you use and your general health.

  • Day of the operation; you should remove jewelry and accessories, and remove any nail polish from makeup and nails.

  • You can stop taking medications that dilute blood, such as aspirin, at least 1 week before under the supervision of a doctor.

  • If you are experiencing ailments such as flu and cold, you should share this with your doctor.

  • Avoid foods and drinks that will cause bleeding before surgery, as herbal teas such as green tea can increase bleeding. (Ginger, turmeric, garlic, red wine, some types of peppers, olive oil)

  • Although women's menstrual periods are not a medical obstacle to surgery, surgery is not suitable in these periods as it may increase edema that may occur afterwards.

  • Since smoking can negatively affect the healing process, the patient should be informed in this regard.

19. Is there a season for rhinoplasty?

Rhinoplasty surgery do not generally have a season. However, it is a problem that the nasal plaster can loosen more easily as a result of sweating in hot weather, but these problems are not experienced because the materials used in recent years provide air permeability. Nevertheless, there are situations that patients with rhinoplasty should pay attention to in the summer.

  • During the wound healing process, there is a possibility that the color of the surgical area may become darker with the effect of sun and tanning. Sunscreen creams recommended by the doctor and a hat should be used to protect against the effects of the sun.

  • Sunglasses, which are mostly used in summer, should not be used after surgery.

  • After the operation, swimming in the sea or pool should be stopped for a period of time recommended by your plastic surgeon.

20. How many hours does rhinoplasty take?

Rhinoplasty surgery vary depending on the operation to be performed. If there is no septum deviation or other problems, the operation may take 1,5-2 hours. If only the tip of the nose is to be shaped, this process becomes even shorter. However, with rhinoplasty, complicated surgeries such as a deformity in the nose, secondary surgery, and reconstructing  a nose with a rib cartilage can take up to 5-6 hours.

21. Will there be a lot of bleeding in rhinoplasty?

Since millimetric procedures are performed in rhinoplasty, bleeding is not desired at all during operation. Trying to do such delicate surgical procedures for a bleeding nose can make the surgeon's job difficult as well as increase the swelling and bruises after the operation. For this reason, the techniques called atraumatic method, which are performed by entering under the nasal cartilage membrane and bone membrane, where the bleeding is the least, are preferred. Performing operations in these areas will reduce bleeding and cause less edema and bruising after surgery. Rhinoplasty surgeries are operations that are attempted to be performed in a bloodless environment.

22. Will there be any scar after rhinoplastysurgery?

Since the open technique used in rhinoplasty is performed by opening a small incision between the nostrils, there is a possibility of a slight scar on this part of your nose after rhinoplasty. Since wound healing varies from person to person, this scar does not look the same in everyone. In the operations performed with closed technique, there is no visible scar that can be seen from the outside.

23. How long will the swelling and bruises that occur after rhinoplasty pass?

Sometimes no bruising is experienced in rhinoplasty, depending on the type of trauma. The swelling and bruising may vary depending on the procedure to be performed. Bruising starts to turn yellow within 1 week and becomes invisible. Swelling and edema are usually at the highest level on the 2nd day after the operation. While most of the edema passes in 1 week in most of the patients, a small part of the edema may last up to 3 months.

24. What is a depression in the nasal bridge?

It is a condition that occurs as the swelling on the back of the nose decreases in cases such as excessive removal of the cartilage and bone that support the nasal ridge in rhinoplasty operation or not well supported.

In cases of nasal collapse, revision surgeries ranging from rib cartilage harvesting may be required.

25. What is drooping nasal tip after rhinoplasty?

It is evaluated as the decrease in the angle between the nose and the lip, the loss of the height of the nasal tip, and the drop of the tip. Especially in septum surgery, if the deviated cartilage is located close to the tip of the nose and after this area is removed, if the nasal tip support is weakened and measures are not taken, it is inevitable that the nasal tip height loss and the nose lip angle decreases. In such patients, the situation can be prevented by applications that support the nasal tip. Sometimes, this angle, which seems higher due to edema in the early postoperative period, decreases a little when the edema decreases, and the patient may consider this as a nose tip drop. Since the edema in the noses performed with closed atraumatic techniques is less and the nasal ligaments are protected, there is no significant change.

The techniques used in rhinoplasty operations are planned to keep the shape of the nose for many years. There is no fall on the tip of the nose after the operations performed by the surgeon with the right technique.

26. Does asymmetry occur after rhinoplasty?

The cartilage and bone structures shaped during the rhinoplasty operation go through a healing process where they reunite with the skin during the healing process. During this process, a shrinkage occurs in the healing area. In this process, if the supports of cartilage structures are not strong enough against these forces, they may undergo a change in shape against the forces of the wound healing process, and as a result, problems such as asymmetry may occur with the recovery of edema. Apart from the support of cartilage structures, it provides a healing process in which these forces are less, such as edema and bruising in the use of atraumatic technique.

27. Will there be a loss of smell after nasal surgery?

Temporary odor problems may occur after rhinoplasty. The olfactory cells are close to the area of the nasal arch. Edema or damage to the mucosa that allows to smell while lowering the nasal hump by cutting or rasping may cause problems in smell. In extremely large arched noses, when the nose is reduced, the mucosa may be compressed in a narrow area. When the nose is lowered, the squeezing or clumping of the mucosa in the narrow area can lead to problems with odor.

28. What should be considered after rhinoplasty?

One of the most important issues after rhinoplasty; It is necessary to pay attention to traumas, and avoid situations and activities that may increase blood pressure in the head area, such as bending over and tying shoe laces.

  • In order to protect your nose from trauma and reduce swelling after the surgery, it is recommended to raise the head with a pillow as much as possible while lying down.

  • Blood thinners and foods should be avoided.

  • Exercise should not be done for a while after rhinoplasty surgery. Normal activities should be avoided for 10 days and heavy sports for 3 weeks.

  • After the operation, care should be taken not to get the plaster or splint that protects your nose wet. It is recommended not to wash the head part in the bathroom for a while after the nose aesthetics.

  • Traumas should be avoided.

  • Glasses should not be used after rhinoplasty surgery for a period of time recommended by your plastic surgeon.

  • Regular nutrition and regular sleep accelerate the healing process.

  • Hard and fast brushing of the teeth can cause sensitivity in the base of the nose

29. What is the cost of rhinoplasty in Istanbul 2024?

Rhinoplasty prices vary according to the procedures to be performed. Depending on the size of the procedure to be performed, operating hours are also prolonged. 30. Should You Massage Your Nose after Having Rhinoplasty?

Nasal massage after nose surgery can help to reduce swelling and make you more comfortable during your recovery. However, not all plastic surgeons will recommend that you massage your nose after rhinoplasty, so it is crucial that you follow his or her specific recovery guidelines. The logic of massage application is the process of increasing the circulation in this area by mechanical pressure. Accelerating circulation can be considered as accelerating wound healing.
